Watch THJ's face, he knows, but is looking for some kind of excuse. He does his job and that does NOT happen.

Nope, it was pure DFS mistake. Beal didn't use the screen, he went the other way. DFS obviously didn't expect the screen (no communication, obvious many times through the game) and got stuck on it. If THJ switched, Wagner would have that lay-up, because DFS was stuck behind him. Freeze screen the moment Beal changed direction and went straight to the basket.

Wright was also a step late for help.

Don´t see it. Wagner is at least 3-5ft behind the 3-point line and the Mavs are up one. He is also slower than DFS or THJ. DFS is clearly not able to follow Beal because Wagner blocks him. He obviously fails to understand what Beal wants to do but that shouldn´t be a problem because he is right next to Wagner and at the very worst could have forced him into a long contested 3. Switch everything means everything and in this case THJ has to pick up Beal.
Agree on the lack of communication aspect. I think that´s the biggest overall problem on defense. The Wizards torched the Mavs with cuts all game long and the Mavs failed  to make the right rotations or switches most of the time.
